| Existing clients: submission of this form is required when requesting MS Environmental Solutions, Inc. services and support. This form should be used to request IAQ assessments, asbestos services, safety inspections, emergency responses, training, and other professional services. The submission process is as follows: |

| Upon receipt, MS Environmental Solutions, Inc. will contact you to make an appointment to evaluate the scope of work. Following evaluation of the scope of work, a cost estimate will be provided for approval. A written notice to proceed will be issued by the District upon acceptance of the cost estimate. |
